    Title: 醫院高危險群作業人員對作業場所危害風險知覺之研究 : 以臺北市某醫學中心為例

    Abstract: 醫療作業人員經常處於複雜的運作環境中,極易因人為疏失或管理不當或防護措施失效,
    進而導致危險有害作業之接觸、漏洩、化學反應,或引發感染、火災、傷亡等不幸意外。醫院
    作業人員若不清楚造成災害源為何,極可能因而造成作業人員以及病人甚至全院危害風險。
    人為因素之影響造成作業風險的主要來源,所以醫院應該重視高危險作業人員危害風險問題,
    藉此來瞭解醫院高危險群作業人員的風險知覺,以改變高危險作業人員正視其環境及作業安全
    健康,並防止危險群作業災害發生。因曝露於醫療危害作業風險之問題,為能使人員了解進而
    改變其作業風險知覺,以維護醫院安全及自身健康,將是急需探討的課題。
    本論文以台北市某醫學中心之高溫作業、噪音作業、游離輻射作業、化療作業、鉛作業、
    特定化學物質作業、供膳作業、廢棄物處理等高危險群作業人員,利用比例分層隨機抽樣,
    共抽取 392 位員工為研究對象。經問卷調查及統計分析,以瞭解醫院高危險群對人因工程安全性、物理性、化學性、生物性及社會心理等作業場所危害的風險知覺為何?進而探討醫院高危險群在安全衛生對策對員工在高危險作業場所的風險知覺影響為何?
    本研究結果為:
    一、醫院高危險群對人因工程安全性、物理性、化學性、生物性及社會心理等五項作業場所之
    危害風險知覺有顯著性差異。
    二、醫院高危險群作業人員之危害風險知覺與安全衛生對策影響因素有顯著相關。
    三、醫院高危險群作業人員之衛生安全對策影響因素對作業場所危害風險知覺具有預測力。
    依據研究結果對醫院高危險群管理提出以下建議:
    為了落實作業危害風險管理,必須針對特定的對象、採用適當的對策以達到降低職業災害。
    醫院相關單位可針對醫院高危險群作業特性上的差異,對不同的作業別,採用不同的安全衛生
    對策及危害預防系統安全品質缺口模式,提升作業品質突破模式中的五個缺口。為建置及改變
    高危險群作業人員之風險知覺以至行為,建議推動安全衛生自主管理策略以確實對作業場所危害風險加強改善。即由自主性環境管理制度,重視工作安全的作業習慣且透過教育、訓練,以提昇人員安全知能,並持續規劃職業傷害防止對策進而有效管理高危險群作業風險。
    Medical personnel usually work in a complex environment where human errors,mismanagement, failure of precaution measures will lead to dangerous operational contact, leakage,chemical reactions and in serious cases, infectious outbreak, fire or life casualties. Negligence or simply unawareness of possible disaster source could lead to catastrophic disasters that could endanger the operation personnel themselves, patients and even the entire hospital. Human errors are the major sources of operational risks, thus hospital authorities should place special emphasis on the education and awareness training of the operation personnel of high-risk groups in order to prevent catastrophic consequences. Due to the reasons that these personnel expose themselves to great risk and the benefits of maintaining a safe environment in the hospital, it is an imperative issue that needs our attention.
    This research conducts random sampling, according to class ratio, of a certain Taipei City medical center with 392 high-risk group personnel as research subjects in areas like high-temperature operation, noise operation, ionizing radiation operation, chemotherapy operation, lead operation, specific chemical agents operation, meal service and waste disposal treatment. Through questionnaire investigation and statistical analysis, an overview of their danger and risk awareness in regards to engineering project safety, physical surroundings, chemicals, biological and social psychology are studied. Further investigation is conducted in regards to high-risk group personnel’s danger and risk perception concerning safety and hygiene policy of hospital.
    The results of the research are as follows:
    1. There is significant difference of danger and risk perception among high-risk groups:
    engineering project safety, physical surrounding, chemicals, biological and social psychology.
    2. There is significant correlation between danger and risk perception and safety and hygiene policy of high-risk group operation personnel.
    3. There is predictability with respect of high-risk group operation personnel’s understanding of safety and hygiene policy and their danger and risk perception.
    The research conclusions provide the following suggestions for high-risk group management:
    To implement effectively danger and risk management, suitable measures must be taken against specific targets. Relevant hospital authorities could develop operation-specific safety and hygiene policies using prevention system gap quality model to improve the operation quality of the five gaps of the quality breakthrough model. Autonomous environmental management is recommended to effectively develop the danger and risk awareness of the high-risk group operation personnel. It places emphasis on secure work environment through education, training, raising awareness and constant professional hazard prevention planning that can effectively manage the operation of the high-risk groups.
