    Title: 以空勤總隊海豚直升機失事事件論直升機飛安改進策略

    Abstract: 民國 105 年 3 月 11 日,內政部空中勤務總隊一架AS365 N3 型直昇機,登記號碼NA-107,機上載有正、副駕駛員、機工長各一員,海巡特勤隊員 2 員,共計 5 員。該機由松山機場起飛,前往位於新北市石門地區海岸之德翔台北貨輪擱淺處,執行 6 名油汙探勘人員之載運任務。該機於接近貨輪準備進行人員吊掛時,墜毀於該擱淺貨輪船身左側之海面上,直昇機全毀,正駕駛員及一名海巡特勤隊員死亡,副駕駛員、機工長及另一名海巡特勤隊員重傷 2 。由於本人退伍於陸軍航空602旅,退伍前職務為空勤檢驗士官長,並曾與該機副駕駛同飛一段時間,深感應以此次事件深入探討,期望能對直升機飛安有所幫助。
    本研究分為六章;分別為第一章緒論,介紹事件經過,飛安會調查方向及本文研究方法步驟。第二章介紹飛機規格、特徵及性能,從中研究飛機性能是否與本次事件有影響。第三章為失事原因分析,個別由天氣因素、機械因素及人為因素探討與本事件之影響。第四章為LTE(尾旋翼效應喪失)介紹,說明LTE原理與本事件相關處。第五章說明人為因素可能對本事件之影響。第六章為結論說明本事件可能肇因,本次研究目的在於找出失事的原因,提出可能避免的方案,不在於責任的歸屬,目的在於避免相似事件再次發生。
    近年來因科技發達,單純因機械故障而造成重大空難已不多見,大部分的空難都與人為疏失有關,如何減少疏失及避免因此造成致命的事故,是提升飛航安全的核心。
    本研究將先透過該機保養單位(空中巴士直升機) 訪談及飛安會事實調查報告收集本事件之事實,研究該機型特性,再收集國際及國內失事調查報告,研究是否有類似狀況,及失事原因。透過現職飛行員及機械員的問卷調查了解目前直升機從業人員對LTE 及人為因素的了解狀況。針對上述研究失事原因加以探討避免及解決方案。期待能對直升機飛行安全有所幫助。
    On March 11, 2016, a National Airborne Service Corps, Ministry of the Interior (NASC) Airbus AS365 N3 helicopter, registration number NA-107, with one pilot as pilot flying, one copilot as pilot monitoring, one crew chief, and two Coast Guard Administration-Special Services Company (CGA-SSC) members on board. The helicopter departed from Taipei Songshan Airport and headed toward the coast of Shimen (New Taipei City) to transport 6 oil spill surveyors, where a T.S. Lines cargo ship ran aground. When preparing to hoist personnel near the cargo ship, the aircraft fell into the sea to the left of the grounded cargo ship. The helicopter was completely destroyed; the pilot and one CGA-SSC member were killed, and the copilot, crew chief, and another CGA-SSC member were seriously injured. Since I serviced in Army aviation as a air crew member, QC Master sergeant, and flight with the copilot for a while. I really concern about this accident should be discussed deeply. Hope I can find some suggestions that will help future helicopter flight safety.
    Due to scientific and technological development improved in recent years, it is seldom to have a serious fatal accident that is simply caused by mechanic failure. Most air accident is related to human error. It is the main core to approve flight safety by reducing human error.
    This research will first visit the maintenance unit of Air bus helicopter CO. and study factual report of ASC and the characteristics of the helicopter. The international and domestic air accident reports will be collected to reveal if there is any similar event happened before. I also make a google form to interview helicopter pilots and mechanics to find out how much they know about LTE and human factor error. According to the studies list above, this research will disclose any possible cause of the event and propose some suggestions to prevent that happens again in the future. Hope the research will help to approve helicopter flight safety.
    This research includes 6 chapters. First Chapter is abstract, will introduces the accident, how ASC will invest the accident and how this research is going. Chapter 2 will introduce the helicopter’s performance, spec. and design. I will study the helicopter performance to find out if any factor is related to the event. Chapter 3 will analyze weather, mechanic and human factor see if they are related to the event. Chapter 4 will introduce LTE (Lost Tail rotor Effectiveness). I will explain the theory of the LTE and how it is related to the event. Chapter 5 will discuss how the possible Human factor is related to the event. Chapter 6 will conclude the possible cause of the event, and will propose some suggestions to prevent the same event happen again.
